Insurance & HMO
Making your diagnostic visit easier.
Confirm your coverage before you come, and bring what you need on the day.
Before your appointment
Confirm your coverage first.
If you intend to use an HMO, an insurer or an employer healthcare arrangement, confirm that your plan currently covers the test you need before your appointment. Coverage arrangements change, and plans differ in which diagnostic services they include and whether prior authorisation is required.
Checking in advance avoids the two situations that cause most difficulty on the day: arriving without an authorisation the plan requires, or discovering a test is not covered under your level of plan.
What to have ready
- The name of your HMO, insurer or employer scheme
- Your enrollee or member number
- Your referral, where your plan requires one
- An authorisation or pre-authorisation code, where applicable
- A valid form of identification
- A contact number your provider can reach you on
